Complex Care Services

Complex care is also known as continuing or long-term care and is a wide-ranging term for what is essentially a condition requiring substantial health & social care support, to enable someone to stay in their own home. The condition could be due to a chronic illness, a disability, a debilitating mental health condition, or a life changing event (accident, medical error etc), and leaves the individual relying on specialist medical equipment to assist with things like: Breathing (eg-ventilators), Feeding (eg-PEG feed tubes), Waste management (eg-colostomy bags), and Moving (eg-lift management or hoist). As such, Complex carers need to be specially trained to provide this type of care

Our multi skilled carers are highly trained We aim to provide 100% of your care needs, no matter how complex your condition is, whilst building strong and lasting friendships with you.  

We work very closely with other healthcare professionals – GP practice, community nurses, physiotherapists, occupational therapists and speech and language therapists, hospital consultants to provide tailored care for those we support. Our clinical governance is robust and staff follow comprehensive support plans and health action plans to ensure that staff know exactly how to support a person to stay healthy and well.
Our aim is to help people live at home and to live a full life. It is important to us that our care programs are tailored to both medical needs and emotional well-being.

We understand that coping with complex health, or medical condition, can be difficult and that psychological and social support is vital in helping overcome feelings of low self-esteem, loneliness and, for some people, depression. Together, we will build a care programme that is all about how we can help and support your loved one. We will liaise with the GP practice, community health teams and hospital consultants to ensure they receive a continuity of care in your home across a complete and informed support team.
End of Life Care, Stroke and Brain Injury

Here at Brook of Life Care we understand that end of life care is one of the most important stages of someone’s life. At this time everyone deserves to be treated with respect and dignity. Our carers receive specialist end of life training and always remain polite and empathetic whilst understanding how difficult this time is for both service users and their families.
The services we offer can range from half hour check up calls right up to 24-hour care. All our staff members are aware that even if the service user can no longer communicate these verbally, they must be adhered to. Whether a long-term or short-term care plan is needed, Brook of Life Care can help provide post stroke care at home.

Our carers can liaise with the therapists involved in your loved one’s treatment, such as physiotherapists and language therapists, ensuring that daily exercises are carried out and prompting them to take medications as prescribed.
Brain injuries are life changing for those that sustain them, and the people closest to them. Having expert support in place can help them manage that change a little easier. At Brook of Life Care, we take a flexible approach to supporting clients with a brain injury to live independently in their own homes. We tailor our services to each individual’s unique needs, choices, circumstances; providing the support they need and designed around them.
